If your account has had no activity in the last 12 months, please print this form, complete and return to avoid a $2.00 per month inactive account charge and protect your account. Historically, inactive accounts are more susceptible to fraud than accounts that are frequently used. In addition, unused accounts become subject to abandoned property laws in most states, including Virginia. The monthly fee is required to offset the special handling costs of these accounts.
